Hiking photographers, what do you use to carry your camera around while hiking?

I discovered the hard way this past weekend just how painful a neck strap with a heavy camera on it can be after I hiked 7 miles that way. My neck/back are still in agony as a result. I’ve considered the shoulder/side type of strap before but I think while hiking it will get jostled around a lot and also it will be in danger of scraping through brush or hitting things while on narrow paths.
